Central 2009 H1N1 Influenza Vaccine Recovery Program

Step-by-Step Instructions

  1. Select H1N1 flu vaccine that is set to expire at the end of June 2010.
  2. To calculate H1N1 doses into pounds, use the calculator below.
  3. Package slightly less than 20 pounds of doses into a box with packing material. In your office, weigh and verify that the package does not exceed 20 pounds.
  4. Complete the packing slip electronically and print out a copy to be placed in the box.
  5. When the package is ready, contact UPS, if you don’t already have a UPS routine delivery system in place.

Packing Slip
You can complete this packing slip electronically.
Note that "Manufacturers" and "Formulation" columns are menus with pre-entered choices, use scrolling arrows to the right of the column to choose manufacturer and formulation. You can type in the first letter of the manufacturer or formulation and the entire name will appear. Click on the field to confirm your choice.
When done, simply print out a copy to be placed in the box.

Vaccine Weight Calculator

You should receive two, pre-paid UPS labels by the week of June 21. To convert vials, syringes and/or nasal sprayers, simply enter your practice's numbers in the "Vaccine Quantity" boxes below. Your total calculation will be automatically converted and at the bottom you will be told if you need to use one or two labels. If you have more than 40 pounds of unused doses, the calculator will tell you to call the Health & Human Services number for specific instructions.
Remember, you will receive a third, pre-paid UPS label in the fall to return late-dated, unused H1N1 flu vaccine doses.
